CLUB REPORTING FORMS

Quarterly Activities Report -
What is it? The form used to report club fundraising, sponsorship and volunteer hours for the quarter.
Where is it sent? Sertoma Headquarters and the District Governor
When is it due? July 10, Oct 10, Jan 10 and April 10.

Quarterly Dues Statement -
What is it? The form used to report club membership totals for the quarter and to determine amount due to the District Governor.
Where is it sent? District Treasurer.
When is it due? July 10, Oct 10, Jan 10 and April 10.

Club President's Goal Sheet -
What is it? The form used to develop the club goals for the year.
Where is it sent? District Governor
When is it due? July 1

Governor's Networking Award Form 2005/2006 (PDF)
What is it? This form is completed to track club participation in this program.
Where is it sent? District Governor
When is it due? July 10, 2006

Club Financial Audit Form (Word Document)
What is it? This form is completed at the end of each fiscal year.
Where is it sent? District Governor
When is it due? July 31

SHARPS Dues Statement
What is it? This form is used to calculate Sertoma Hearing Aid Recycling Program dues for each club. Payment of dues enables clubs to participate in the recycling program.

Matching Grant
What is it? The community service matching grant program was developed to help clubs complete sponsorship projects that may be above and beyond their normal capabilities. Grants may be requested for up to $5,000. The total dollar amount allowable is based upon the amount the club is able to raise for this project. The club may apply for $1 of grant money for every $2 raised by the club, i.e., a $5,000 grant requires the club to raise $10,000. The total amount allowed is based upon the club’s membership in the Sertoma Foundation. See PDF file (link above) for full details.

CLUB AWARDS - Every club should review these awards at the beginning of the year to develop a plan to achieve these awards.

Club Achievement Award -
What is it? The form used to report club activities for an entire year. Emphasis is placed upon Foundation participation, sponsorships, membership and hours of community service. See page 26 of the Awards and Reports Section of the Club Manual for full details
Where is it sent? Sertoma Headquarters, International Director and the District Governor
When is it due? July 10

Club Excellence Award - Due to District Governor by July 1
What is it? The form used to report club activities for an entire year. Emphasis is placed upon Foundation participation, sponsorships, dollars raised and hours of community service. See page 26 of the Awards and Reports Section of the Club Manual for full details
Where is it sent? Sertoma Headquarters, International Director and the District Governor
When is it due? July 10

Honor Club & Distinguished Honor Club Award
What is it? The form used to report club activities for an entire year. Emphasis is placed upon membership retention and recruitment. See page 28 of the Awards and Reports Section of the Club Manual for full details. In order to qualify for this award, a club must also meet the requirements of the Club Excellence Award.

When is it due? July 10

INDIVIDUAL AWARDS

Gem Award
What is it? An award presented by a club to a recognize new members upon completion of the assimilation process which includes sponsorships and recruitment of at least one additional member. See page 6 of the Awards and Reports Section of the Club Manual for full details

Mentor Award
What is it? An award presented to a recognize a member for assisting new members through the first year of membership and completing the GEM Award Process. See page 16 of the Awards and Reports Section of the Club Manual for full details
